Referral Process
At Cheshire Young Carers we strive to
make the referral process as simple as possible. Please read the
following guide if you have someone you would like to refer to the
project.
Step one
If you are a professional working with a young person who takes
on a caring role at home or if you are a parent who wants to refer
your child you need to fill out an initial referral form. This is
available as a download
and requires only your contact details and those of the young
person.
Step two
Following this process, you will receive a phone call asking for
more information about the young person i.e. what school they
attend; if any other services are working with the family. This
should take no more then 10 minutes.
Step three
Next the family will be contacted to arrange a home visit. The
nature of the caring role the young person has taken on will be
assessed and the family will be provided with any information they
may need.
Waiting list
Please be aware there is a considerable amount of interest in
the project, which may require going on a waiting list to receive a
home visit. It is important that you do not state any time frame to
the young person prior to us contacting them.
Notify the family
It is essential the family are aware the referral has been made
and why, as we will discuss this with them. This process can be
sped up by supplying a phone number that makes it
easy for us to contact you.
