About the Role
As a Nurse Practitioner at St John Ambulance, you will work as part of a team to aim to reduce the number of homeless frequent attenders in A&E by providing street and hostel triage services. Your key responsibility will be to provide specialist, clinical, social and holistic care undertaking appropriate clinical assessment and interventions, increase vaccinations, screening and treatment of blood borne viruses, refer to community based services using clear and effective pathways.
You will use your expert nursing skills to triage and treat members of the homeless community, making referrals to primary care services and ensuring reports are written into the Plan of Care as well as liaising with other healthcare professionals either in person or by phone where needed.

About You
To be successful as a Nurse Practitioner, you will bring extensive experience as a registered Nurse and a proven ability to make clinical decisions both as part of a multi-disciplinary team and when working alone. You will have excellent communication skills to create and develop relationships with external agencies and partner organisations. You will be passionate about the care, dignity and individuality of people to be successful in this role.
You will be required to be flexible and will work both daytime and evening shifts over a 37.5 hour week.
